Click on the link below and then choose “sign in or sign up” from the top right hand corner. Every section on the intake form needs to have something in it — even if it is just a “n/a” to move forward through the intake. To schedule a first office visit and establish care, you will need to call us to get that scheduled. Call us if you have any questions 406-607-3060.
What to Expect
Thank you for choosing The Natural Path! We know you have choices when it comes to your health care and we are dedicated to providing patients with the best care possible.
Before your scheduled appointment, you will need to open an account in our Jane App and fill out intake forms. There is a link above. We know your time is valuable and completing your patient forms at least two days before your appointment with Dr. Raish will maximize the amount of time spent with the him. It is also important have any lab work you want him to review also turned in at least two days before your appointment or he might not be able to go over those with you until a later appointment.
Your first visit will be a thorough assessment of your health, going over any previous lab work, and time will range from 60-90 minutes. If you are unable to keep your scheduled appointment time for any reason, please let us know as soon as possible so that we may reschedule your visit.
We accept all forms of payment but do not bill for insurance. We will however provide each patient with a superbill to submit to their insurance company upon request. It is not our responsibility to see that you get reimbursed from your health insurance. Please check with your insurance provider for coverage before your appointment if they cover “out of network” services from Naturopathic Physicians. Full payment is due at time of service.
